11/10/2023 EnchantaVaughn Approved Behavior: Josiah Initially seemed in a positive mood at the start of session, but he later informed QMHP-C that you have been involved in a physical altercation with a peer in the community one times with moderate intensity during a 2 to 3 minute incident. Client reported this was the same peer who punched him in the back as he was riding his bike a couple of days ago. Client reported the peer threatened him and got in his face and that caused him to choke slam the peer one times with moderate intensity during the 2 to 3 minute verbal/physically aggressive incident. Behavior: Mother reported client continues to struggle with managing his anger on a daily basis with 2-4 minutes in duration at which he will act out by throwing temper tantrums when he cannot have his needs met. Intervention: QMHP-C probed client as to his request for todayâs individual counseling session. QMHP-C then assessed clients mood as well as redirected him for switching his move from negative to positive at the start of session. QMHP-C then use a psycho educational activity with client that allowed him to focus on controlling his emotions as well as calming down and participating in session. QMHP-C used active listening as client openly spoke about the incident as well as the issues heâs been having with this peer over the past couple of weeks. Intervention: QMHP-C began the session by inquiring about Josiah's emotional well-being. QMHP-C used active listening and positive feedback to encourage Josiah to express his feelings and clarify his actions to why he was so upset. QMHP-C assisted Josiah on brainstorming the issues he had had at home. QMHP-C reviewed Josiahâs behavior at home and if he had experienced any symptoms of anger and anxiety at home. QMHP-C assisted Josiah in expressing how he was acting and behaving at home when asked to do something or not getting his way. QMHP-C used active listening and supportive statements to encourage Josiah, as he shared his thoughts on how he was treating his family Response: Josiah seemed receptive to todayâs individual counseling session evidenced by his ability to verbalize understanding of session. Client reported this is the same peer they punched him in the back the other day and seems to be the bully of the neighborhood. Client reported this person knows he is not soft or a punk and knows he will always defend himself in an aggressive manner. Client reported as he choked slammed the peer the began to ask for the help of other peers to get him off of him. Client reported once he let the pier go the peer ran home and acted like a girl. Response: Josiah was able to participate and discuss his triggers, he expressed that he enjoyed hanging with QMHP-C and learning ways to not get so angry and mean, evidenced by âI think that when you make me explain how I am feeling, I am understanding that I canât react in angerâ, comment. Josiah was able to participate in therapeutic conversations with QMHP-C on not saying mean and negative things. QMHP-C discussed the session with Josiahâs father and asked him for some support to encourage Josiah to follow directions and do what he was being asked.
